Jason was born on January 3, 1979, in Phoenix, Arizona to Dale Joseph Lewis and the late Patricia Trantow Lewis. He was in sales in the retail business.

Survivors include; father, Dale Joseph Lewis of the home; sister, Teara R. Alford of Nashville, NC
